Geriatric Day Hospitals in Winnipeg

There are five Geriatric Day Hospital Programs in Winnipeg. 

Day Hospital Program: A program that provides assessment and rehabilitation services to promote healthy living for elderly clients in the community. A variety of health care professionals trained to work with older adults, 65 and over, are available. This includes Occupational Therapists, Physiotherapists, Nurses, Social Workers, Geriatricians, Dieticians, etc.They utilize a team approach and attendance is usually once weekly for a short-term. Transportation can be arranged for a small fee.

Your physician has to be in agreement with the referral. Talk to your doctor if you think this is a program for you.

The Day Hospital referral form is filled out by the following people: Any primary health care provider. Referrals from health care professionals should be forwarded to the primary care provider for their agreement/approval. Hospital Inpatient Units, Emergency Departments and Geriatric Programs.

The Home Care program was established in 1974 to help people live at home, remaining independent for as long as possible, thereby avoiding or delaying the need for individuals to go into long term care facilities.

The mandate of the program is to provide effective, reliable and responsive community health care services to support independent living, develop appropriate care options with clients and/or family and facilitate admission into long term care facilities when living in the community is no longer possible.

Programs:  Home Care, Dialysis and Ostomy Care, Community Intravenous Therapy, Self and Family Managed Care, Respiratory, Children's, Palliative Care

Services:  Personal Care, Nursing, Counselling/Problem Solving, Household Assistance, Respite/Family Relief, Occupational Therapy Assessment, Physiotherapy Assessment, Assessment for Personal Care Homes, Supportive Housing, Adult Day Programs, Referrals to Other Services.

Virtual Hospital Ward Pilot Project

Wishes can come true.

89-year-old Winnipeg resident Terry Miller is able to stay at home longer and have less visits to the hospital thanks to a year-long pilot project, which is a type of virtual hospital ward.  She lives with her husband Dan and requires 24-hour care and support due to mini-strokes and Alzheimer's Disease.  Terry Miller is one of ten chronically ill people who have benefited from this virtual hospital project.
