Geriatric Day Hospitals in Winnipeg

There are five Geriatric Day Hospital Programs in Winnipeg. 

Day Hospital Program: A program that provides assessment and rehabilitation services to promote healthy living for elderly clients in the community. A variety of health care professionals trained to work with older adults, 65 and over, are available. This includes Occupational Therapists, Physiotherapists, Nurses, Social Workers, Geriatricians, Dieticians, etc.They utilize a team approach and attendance is usually once weekly for a short-term. Transportation can be arranged for a small fee.

The Day Hospital referral form is filled out by the following people: Any primary health care provider. Referrals from health care professionals should be forwarded to the primary care provider for their agreement/approval. Hospital Inpatient Units, Emergency Departments and Geriatric Programs.
